Small-Group Ratios

From one-to-one support to community groups of just two to four people, our services stay small on purpose. People get real attention, not a seat in a crowd.

Person-Centered Planning

Every participant has an individual plan built around their own ISP goals, interests, and dreams, and we update it together as they grow.

Trained, Background-Checked Staff

Every DSP completes fingerprint background checks plus training in CPR/First Aid, crisis prevention, and HIPAA before working with participants.

Door-to-Door Transportation

Wheelchair-accessible vans pick up and drop off at home across our service area, so getting to program is one less thing for families to solve.

Our Approach

Creekstone Care was born from one family's love for their brother, Michael, and from the belief that people with disabilities deserve the very best care and opportunities. This work isn't new to us: our family and team have been serving people with IDD since 1986.

Our days are active and purposeful. Participants choose from activity clubs, practice real skills in real places, volunteer with organizations that count on them, and build friendships that carry into evenings and weekends. Our staff are trained to support communication, independence, and choice. The goal is never to do things for people, but with them.
